Welcome! When you arrive for your first appointment, you will be expected either to turn in your pre-completed forms (linked below) or fill them out after checking in. Dr. Reinke and staff will then conduct a complete eye exam including refraction and dilation to identify any eye pathology and to help design a treatment plan that is tailored to your specific needs.
